I'm blending a few aspects of different PA media, well except magic. But if you wanna be a nit picker some mutations might be considered magic. The setting actaully started out as an idea for a series of short stories spanning a thousand years or so.
About a century before the whole world gets blown up there is an oil blight. Terrorism meets eco-terrorism, figured it would be a way to jump start funding for fusion and renewable power sources. By the time the collapse happens fusion power plants can be scaled down to the size of a tractor trailer.
A lot of things have gone to pot, almost literally. Hemp was started to be used as biomass for methane (for cars and home heating). Also the oil has some handy uses, especially with crude oil being gone. One development was a high strength, light weight plastic. Think of something as thin as Mylar being as strong as a 1/2 inch piece of steel plate.
On advancement was being able to send out probes with a FTL drive.... Oops. Think of a xenophobic space based North Korea' air space being violated. No aliens were sent but they send back some nasty bio-weapons because of some human DNA found on the probe. Most of the Population of the world lives on coast lines, was the same on the alien world. So bye-bye to civilization east of central Pennsylvania and west of Palm Springs. The bombs weren't nuclear so no radiation but they kicked up en ugh dust for a few years of nuclear winter.
People started getting sick and dying. Some lived, and some sort of changed. Instead of ripping apart the proteins in the DNA it bonded with animal DNA with critters that were close to them. So there are mutants that are dog people, cat people, raccoon people . The first generation these mutants are the family members and neighbors.... but after a few generations you get folks on both side who are douche bags towards the others.
The Great Lakes Area of the US got spared the bombardment but had to deal with the starving winter and illnesses.... I live by the Lakes but that wasn't the reason why I made them survive. The mirco-climate here provides fro some good wine (in real life). There are also salt mines under the Lakes. We're the rust belt but there is still a good bit of industry here (as well as the canals of yesterday, rail lines, rivers....). We also have the Cleveland Clinic with is actually a pretty big name in the medical field.
So far I have just been focusing on the South Shores of the Lakes, but the Canadian Side is splendid country as well. During the summer it is easy to sail either way. In the middle of the winter there are some buts who do ice boat stuff and the lakes have been known to freeze all the way across.
The cities on the Lakes start trading with each other soon after the long winter ends. Eventually they become a cross between the Venice City States and a new Roman Empire. As time goes on there allows for some nice politics to enter.
That's about what I am planning in a nut shell.
It provides a decent amount for exploring, fighting, politics, trade missions and the like.
Oh and the coasts aren't deserted. The Freaks rule the ruins. Imagine 7ft tall mutants that are as big a brick as the Incredible Hulk but are a bit developmentally disabled as a retarded jr high schooler. Once the PCs find that out it will be a moral catch 22. Sort of like when Max finds out that Baster is some sadistic steroid induced killer.
